Supported Living

Supporting People to Live More Independently

Access24 Care Services Ltd provides supported living services for individuals who require assistance to manage aspects of everyday life while maintaining greater independence within their own home and community.

We recognise that independence means different things to different people. For one person it may mean learning to prepare meals independently, while for another it may involve managing money, developing social relationships or accessing employment and community activities.

Our role is to provide the right level of support without taking over. We work alongside individuals, encouraging them to build skills, make decisions and remain actively involved in their own lives.

Supporting People to Do More for Themselves

Supported living should enable rather than restrict. Our approach focuses on strengths and abilities alongside identified support needs.

We encourage people to take part in everyday activities, make decisions and develop skills at a pace that is appropriate for them.

01
Start With Strengths We recognise what the person can already do and build support around existing abilities.
02
Set Personal Goals Individuals are supported to identify meaningful outcomes and goals that matter to their everyday lives.
03
Build Skills Support is provided in a way that encourages learning, confidence and gradual skill development.
04
Review Progress Support arrangements can be adapted as the person's abilities, needs and aspirations develop.

Getting Started

Arranging Supported Living

We work to understand the person and the support required before agreeing an appropriate arrangement.

01
Initial Conversation

Contact Access24 to discuss the individual's circumstances and the support being considered.

02
Assessment

We seek to understand needs, strengths, preferences, risks, communication and desired outcomes.

03
Support Planning

Support arrangements are developed around the individual and what they want to achieve.

04
Review & Development

Support is reviewed as circumstances, skills, needs and personal goals change.

Safe Support

Balancing Independence With Safety

Promoting independence does not mean ignoring risk. We aim to support people to make informed choices while identifying and managing risks proportionately.

Our approach includes person-centred care planning, safeguarding awareness, clear communication and appropriate risk management.

Individuals remain central to decisions about their support wherever possible, and arrangements are reviewed when circumstances change.
