Adults Aged 18–65

Individual Support for Everyday Life

Access24 Care Services Ltd provides support for adults aged 18 to 65 who may require assistance with aspects of daily living, personal care, health, community participation or maintaining their independence.

We recognise that adults within this age group may have very different circumstances, abilities and ambitions. Support therefore needs to reflect the individual rather than their age or diagnosis.

Our approach focuses on understanding what the person can do, what support they require and what they would like to achieve in areas such as everyday living, relationships, education, employment, wellbeing and community life.

Your Goals

Supporting You to Live Your Own Life

Our support aims to enable rather than take over. We encourage individuals to remain actively involved in their own routines, decisions and future plans.

Goals are personal. For some people this may involve becoming more independent at home. For others, it may involve developing friendships, accessing education, taking part in activities or becoming more confident in the community.

01
Identify What Matters We listen to the individual's priorities, ambitions, preferences and desired outcomes.
02
Build on Strengths Existing abilities are recognised and used as the starting point for support and development.
03
Develop Confidence Support encourages people to try, participate, make choices and develop greater confidence over time.
04
Review Progress Support can be adapted as circumstances, abilities and personal goals change.

Getting Started

Arranging Support

We aim to understand the individual and their circumstances before appropriate support arrangements are agreed.

01
Initial Discussion

Contact our team to discuss the person's circumstances and the type of support being considered.

02
Understand Needs

We consider needs, strengths, preferences, communication, risks and personal goals.

03
Plan Support

Support arrangements are shaped around what the individual needs and wants to achieve.

04
Review & Adapt

Support is kept under review as needs, circumstances and personal outcomes change.

Safe & Respectful Care

Promoting Independence Safely

Supporting adults to make choices and become more independent involves understanding and managing risk appropriately.

We aim to balance individual choice and positive risk-taking with appropriate safeguards, person-centred planning and safe working practices.

Safeguarding concerns are taken seriously, and individuals are encouraged to speak openly about any worries or concerns relating to their care.
